When to use botox? Cosmetic and Medical
Are you bothered by wrinkles around your forehead of your eyes? Than botox can be a very good treatment for you. Our Cosmetic Doctor looks closely at the movements and proportions in the face, and very carefully injects botox into the desired muscles. As a result the wrinkles will soften without disturbing the facial expression. We also use botox on area that seem less obvious. For example, we can reduce a gum smile or soften dents around the chin with Botox. This enhances your natural smile and appearance without other people noticing it.
Next to cosmetic reasons we also use botox for medical purposes, like teeth grinding, migraine and excessive sweating. By properly mapping out exactly what the problem is, when it bothers you and what the intended result is, we try to draw up the right treatment plan together with you so that you can resume your daily activities carefree.
How does botulin toxin work exactly?
The active ingredient of botox is Botulinum Toxin. Botulinum Toxin is a substance used to give a temporary relaxing effect on muscles. It is a natural substance made by the bacterium Clostridium Botulinum and has been used in medicine since the eighties to relax spastic muscles, among other things. It is also used to tackle chronic headaches and migraine. In medical cosmetics it is mainly used to locally reduce muscle activity. For example, it softens the wrinkle formation of specific areas such as the frown line or the crow’s feet. Also called expression wrinkles or mimic wrinkles.
The first time botox will work for approximately 3 to 4 months. If you use Botox more often, this will be an average of 4 to 6 months. Because the muscles under the wrinkle relax, the skin will be smoothed out and the skin will continue to recover in a few weeks to months, so that the wrinkles will fade further and further.
In some situations we do not completely get rid of the wrinkle with botox. The wrinkle can be too deep and the skin can be too damaged. In that case we often combine botox with fillers and skin treatments, so we can still achieve the desired result.
Botulinum toxin or fillers?
Botulinum toxin and fillers are two different types of injectables that are both used for completely different goals. Botulinum toxin is a muscle relaxant that can soften facial expression lines. Fillers are volumizers that can give volume back to the face. You should think of reducing wrinkles, or restoring large contours such as with a liquid facelift. It is also possible to make small corrections with fillers, such as filling the tear ducts, a small nose correction or a lip augmentation.
Depending on your wishes and expectations we will discuss in detail with you during a free consultation whether a treatment with botulinum toxin or fillers is the most appropriate treatment. Sometimes a combination treatment is advised, where we use both botulinum toxin and fillers.
Reducing forehead- or frown lines with Botulinum Toxin
Lines on the forehead or frown lines can be treated very well with botox. Botox, also called botulinum toxin, is a product that reduces muscle activity. You often use these muscles unconsciously, so you do not realize that you are creating these lines. For example, you use the frown muscles not only when actively looking angry, but also during attentive listening, peering into the sun or looking at your computer screen. By treating the frown we create a nice open look, where we see that the eyebrows are slightly lifted.
Wrinkles on your forehead arise because you raise your eyebrows. These lines can be treated very well with botox. By giving these muscles some relaxation, we create a nicely rested and smoother forehead. However, it can happen that you unknowingly raise your eyebrows because you have slightly sagging skin of the upper eyelids. In that case, you should always check with the Cosmetic Doctor whether treating your forehead is the right treatment for you. I this is the case, the sagging skin of your upper eyelids may become more noticable when the lines on your forehead are softer. This can make it cosmetically less attractive.
Eyebrow lift with Botulinum Toxin
A popular treatment is the eyebrow lift by using botox. Drooping eyebrows give a tired look or angry expression. It Is often a combination of multiple muscles. Botox relaxes the muscles that pull the eyebrows down, which lifts the eyebrows slightly.
For which wrinkles is Botulinum Toxin suited?
Wrinkles that are being caused by frequent tensing of the muscles are most suited or botox. For example, people that frown a lot or raise their eyebrows. At a certain point these wrinkles will become visible and won’t disappear anymore, even if you no longer make this movement. By temporarily not activating these muscles, the skin can restore and the wrinkles will be less visible.

How long does Botulinum Toxin work?
The first treatment will be effective for 3 to 4 months. At around 4 to 6 weeks the effect will be maximum. With a following treatment the effect will usually last longer, until about 6 months.Best is to plan a followup treatment after 3 to 4 months, when the botulinum toxin has not yet fully worn off.

Bruxism
Teeth grinding, also called bruxism, is a very annoying problem that can cause damage to the teeth and can lead to serious problems. It is often caused by excessive tension in the masticatory muscle. By injecting a dosed quantity of botox in the masticatory muscle, we can reduce the excessive tension in the jaws without experiencing discomfort while eating. In result the teeth will be better protected.
Migraine
Chronic headaches or migraines are a huge burden on your daily functioning. No fewer than one in six Dutch people regularly suffer from headaches, which hinder you both physically and mentally. In many cases this concernes migraines and involuntary muscle contractions are often the cause of the headache. Rest and painkillers do not always give sufficient effect.
When rest and painkillers don't help, or just to a limited extent, a treatment with botox might be the solution for you. By relaxing the muscles with botox the involuntary muscle contractions disappear, as a result of which migraine is not triggered or is triggered to a lesser extent. Because of this both the physical mental burden will be much less and you will experience less headaches.
Excessive perspiring
Many people suffer from excessive sweating on the forehead, palmes or the armpits. This can cause a social threshold for social activities or for example having a presentation at work. Botox has proven to be very effective at combating excessive sweating, also called hyperhydrosis. By superficially injecting multiple small drops of botox in the upper layer of the skin, the botox blocks the nerve that controls the sweat glands. As a result the glands can produce less to no sweat.
After a botox treatment for hyperhidrosis the effect will be noticeable after about 2 weeks. The result will last approximately 4 to 6 months. You don't have to be scared that the sweat will increase on other areas on the body. This won't happen. This allows you to enjoy the result carefree.

Frequently asked questions about botulinum toxin treatments
During the first intake we will determine whether a botox treatment is suitable for you and what you may expect as a result.
We ask that you do not apply any camouflaging creams on the ares to be treated the day of the treatment. The treatment is usually scheduled for 15 minutes. The treatment itself will only take a few minutes up to ten minutes. The botox will be injected with a super thin injection needle. The fluid that we use for this is bacteriostatic NaCl. In addition, the pain is much lower than you are used to (elsewhere). The doctor is specialized in treating people with a fear of needling, he/she is very calm and takes time for you, in case you are really nervous. She performs the treatment at a leisurely pace, so that the fluid is very carefully introduced into the muscle. The calmer this goes, the less you feel this. You can safely take a painkiller an hour before the treatment, but this will usually not be necessary.
After the treatment you can immediately resume to your daily activities. On the ares that are treated some red or blue spots may be visible or the skin can be locally a bit swollen. This usually disappears within 30 minutes. If needed we can camouflage these with a medical camouflage. Usually this is not necessary. Blue spots are rarely seen. It is important to prevent pressure on your head the first 4 hours after the treatment. You are requested not to lie down or bend over for the first 4 hours after the treatment to prevent spreading to other muscles. You should use the treated muscles intensively to accelerate absorption in the right muscles (smiling a lot, frowning, raising the eyebrows or squinting the eyes). Make sure the treated area is being touched as less as possible within the first hours. After 2 hours you can apply makeup and cleanse the face. The first days you should avoid strong pressure on the treated areas. Sports are allowed again the day after the treatment.
The effect of botox occurs two days to two weeks after the treatment and lasts for several months. After that it is no longer working. The first treatment is effective for 3 to 4 months. The maximum effectiveness lays between 4 to 6 weeks. With a following treatment the effectiveness will be longer, up to 6 months. Best is to plan a follow-up treatment after about 4 to 6 months, when the botulinum toxin has not yet fully worn off. Of course we can not give any guarantees.
